  2. Doctoral Program of Studies-Word version-10-22-09.doc (should only include the 54 hours being counted toward your degree and now includes the Research Residency Form)
  3. Comprehensive Exams Results
  4. Dissertation Faculty Committee (4 members' signatures)
  5. Prospectus Approval (must accompany copy of Human Subjects Approval & corrected copy of proposal)
  6. Candidacy Form
  7. Intent to Graduate (must be submitted at the beginning of the semester expecting to graduate; check deadline)
  8. Dissertation Defense Announcement (must be submitted to COE Graduate Director's office at least one full week before the date of the final defense examination)
  9. Results of Final Dissertation Defense
